Ontonagon Free Immunization Clinic:

Thursday, Aug. 26 from 1-4 p.m. at the health department office, 408 Copper Street, Ontonagon. Call 884-4485 for an appointment.

Required for all children entering kindergarten, all 6th grade students, and all children changing school districts:

  • 4 doses DTAP (Diphtheria, Tetanus, Pertussis)
  • 4 doses Polio
  • 2 doses MMR (Measles, Mumps, Rubella)
  • 3 doses Hepatitis B
  • 2 doses Varicella (Chickenpox)

 

Required for all children 11- 18 years of age who are changing school districts or who are enrolled in 6th grade:

  • 1 dose of Meningococcal
  • 1 dose of Tdap (Tetanus, Diphtheria, Acellular Pertussis)
